West Plano Family Medicine
For over thirty years, West Plano Family Medicine has provided quality medical care for the entire family. Our providers are Jeffrey M. Shade, M.D., Susan D. Ponder, M.D., who are both board-certified Family Medicine physicians. We also have Carolyn Martin, RN-FNP, Jeanne Kilgard, PA who have both been providing medical care 10 + years.
West Plano Family Medicine offers preventive care as well as manages many illnesses, injuries, and chronic conditions. We offer a number of in-office services including blood draws, cardiac stress testing, echocardiograms, holter monitors, allergy testing, skin biopsies and mole removal, joint injections, pulmonary function testing, immunizations and electrocardiography. We offer same day appointments, Monday-Friday and we have a physician on call after hours and on weekends, 24 hours per day, 7 days per week. Our office recently implemented a computerized health record to ensure that our patients will have access to a patient portal where they are able to review lab results, make any changes to personal information such as insurance or address, requests appointments, and receive communication from our office via email.

Our office hours are Monday-Thursday 8:00 a.m.-5:00 p.m. and Friday 8:00 a.m.-4:30 p.m. We accept most insurance plans.

Website looks great, however where does we customers sign up??? Great concept however the web designer needs to more thought into having a place for customers to sign up to receive this elusive "Perks Card".
Hi Jolynn, You can visit our most of our participating businesses to pick up your buy local Perks Card. Visit West Plano Family Medicine in Plano or The House Cafe in Richardson. They have cards available.
